Our Operational Framework: Centers of Excellence
The Alliance represents the full continuum of home-based care, uniting professionals in home health, hospice, palliative care, home care, and Medicaid home-and community-based services. Alliance members have access to dedicated resources around each of these service lines.
The Alliance also has resources dedicated to specific patient populations, including children (pediatrics) and veterans, whose care spans all service lines.
The Centers of Excellence the Alliance supports include:
- Home Health Care: Skilled nursing, physical/occupational/speech therapy, and medical social services aimed at managing conditions or recovering from injury/illness.
- Hospice Care: Specialized care for individuals at the end of life, focusing on comfort and quality of life.
- Medicaid Home & Community-Based Services: Medicaid-supported services and supports provided to individuals in a home or community setting.
- Palliative Care: Patient- and family-centered care that prevents and relieves suffering, which can be provided alongside curative treatment.
- Home Care: Assistance with daily living, companionship, medication reminders, and homemaker services to support independent living.
For each of these Centers, the Alliance provides:
- Advocacy for supportive federal policies and reimbursement
- Regulatory and compliance guidance and resources
- Education and workforce development
- Member communities and connection
- Research, data, and innovation
- Partnerships and opportunities for engagement
