About Medicaid HCBS

Medicaid home and community-based services (HCBS) are a wide range of services and supports provided to individuals in a home or community setting.

In Medicaid, HCBS are available to many individuals with significant health conditions and/or functional limitations and cover a wide range of supports such as home health, rehabilitation, transportation, nutrition, employment supports, and many others. Services provided by members of the Alliance are often both medical supports, such as home health and private duty nursing, and non-medical services such as personal care, homemaker, and chore services.

Each state crafts their own unique Medicaid program that operates under a broad, flexible, Federal framework. The HCBS that states most frequently use are established through a variety of authorities in the Social Security Act, each with their own specific services, requirements, and limitations. Major authorities include:

  • Section 1915(c) HCBS Waivers
  • Section 1915(i) HCBS State Plan Option
  • Section 1915(k) Community First Choice
  • Section 1115 Research and Demonstration Waivers
  • Section 1905(a)(7) Home Health Services
  • Section 1905(a)(8) Private Duty Nursing Services
  • Section 1905(a)(24) Personal Care Services

Medicaid at the National Alliance for Care at Home 
The National Alliance for Care at Home supports members through advocacy, education, research, and publications that seek to advance the provision of care at home and improve the quality of HCBS delivered to Medicaid-eligible individuals. Guided by our Medicaid Advisory Council, we produce member-driven resources to support home care providers.
