UF Health Shands HomeCare offers a variety of programs individualized for every patient. Our Pathway Programs help with disease management (i.e. heart failure, diabetes, COPD); orthopaedic recovery; wound care; IV therapy; medication management; fall risk assessment; caregiver assistance and education; and the overall improvement of the well-being of our patients.
UF Health Shands HomeCare offers a wide array of support services and assistance. Many of these services are covered. Many of these services are covered under health insurance plans and Medicare. The agencies provide services for patients in 16 counties in North Central and Northeast Florida.
