Established in 1966 Andwell Health Partners provides residents of Maine with high quality home health, hospice, palliative care, behavioral health, transitional care, and caregiver services. We are Medicare certified and accredited by the Accreditation Commission for Health Care, Inc. Andwell Health Partners is Maine’s largest independent home health agency with a service area encompassing all 16 counties in the state of Maine.

Skilled Home Care Services – Andwell’s Skilled Home Care Program offers a full range of high-quality nursing, therapy, social work, personal care, specialty and volunteer services, all tailored to meet the needs of patients as they recover from a short-term illness or injury. Patients must meet Home Care requirements for Skilled Services – must be homebound and have a skill need that requires home care. Our home care services include:

Hospice Care – Whether you live in your own private residence or in an assisted living or nursing facility, Andwell’s hospice care brings together medical, psychological, social and spiritual services to address your individual needs.

The patient’s Attending Physician, assisted by Andwell’s Medical Director, oversees the patient’s care plan and the hospice medical services we provide. This includes a primary nurse, hospice social workers, home health aides, chaplains, hospice volunteers, bereavement staff and volunteers.

While the patient is receiving hospice services, there are four levels of care that can be provided: Routine Home Care, Respite Care, Continuous Care, and General Inpatient Care.

Hospice House – The Hospice House provides hospice patients and their families intensive palliative interventions and support, beyond what can be effectively provided in their home or long-term care facility. Our skilled team of medical professionals and specialty trained hospice staff is committed to working with each individual and family to develop a plan of care that reflects patient and family wishes and provides the highest quality of comfort care. www.andwell.org/OurServices/HospiceHouse.aspx

Caregivers – Andwell’s Caregiver Program gives you the flexibility to purchase quality home-based non-medical care and to assist you at home. Caregiver services are flexible, cost-effective alternative to long-term residential care. Personal Support Specialists (PSS) provide services, based on what you need. Personal Care – assistance with bathing, dressing, exercise, and other daily personal activities. Homemaking – help with household tasks like meal preparation, house cleaning, and laundry. Companionship – to ease a person’s isolation, the supportive presence of a companion for conversation, reading, or playing games. Respite – assistance when families and other caregivers need a well-deserved break from the demands of care. Transportation – Door-to-door escorted transportation to medical and other appointments. Household Bill Payments – help with paying routine household bill payments.

Bereavement/Grief Support Services – Andwell’s bereavement staff and volunteers offer support to family members of hospice patients for 13 months following the loss of a loved one, and provide:
