Together with Alliance members and generous supporters, the National Alliance for Care at Home Foundation’s Lighthouse of Hope program has created meaningful moments of comfort, connection, and dignity for individuals and families receiving hospice care. These stories highlight the impact of compassionate experiences made possible through this collaboration across the country. 

Stories are shared with permission from patients, families, and Alliance members. Names may be abbreviated or changed to respect privacy. 

Victor

Caregiver and loved one sharing a joyful moment at a restaurant.

Victor is receiving hospice care and lives with a rare diagnosis. Despite the challenges of his illness, his wish was to spend meaningful time with family. Lighthouse of Hope helped make that possible by supporting a special trip to Disneyland, where he was joined by his sister, who traveled from Germany to be with him. The experience created cherished time together during an incredibly meaningful season.  

Connard

Connard is living with advanced cancer and is now bedridden, receiving hospice care at home.

Simple moments like going out to dinner have become nearly impossible.

That’s where Lighthouse of Hope stepped in.

With support for transportation and a special evening out, Connard and his wife were able to leave the house, enjoy a beautiful drive through the countryside, and share a meaningful dinner together.

“He had a smile on his face and truly enjoyed the evening.”

What may seem like a small experience became something much more—a chance to pause, reflect, and create a lasting memory together.

Sean

For families facing life’s most difficult moments, compassionate care makes all the difference. Thanks to the generosity of supporters and partners, the Lighthouse of Hope helped make a final wish possible—providing flights so Sean’s loved ones could be by his side. In his final days, he was surrounded not by distance, but by family, connection, and love—an impact that will be felt far beyond a single moment.

Hannah

Care at Home support for individuals with health challenges.

Hannah, a young patient receiving pediatric hospice care, had one special wish: to enjoy a joyful birthday filled with experiences she could share with her family.

Through support from Lighthouse of Hope, her family was able to rent a wheelchair-accessible van for several weekends, allowing them to create meaningful outings together.

“Making this little girl’s birthday as big as her heart.”

Over multiple trips, they enjoyed a salon day, visits to the park, and a birthday celebration surrounded by loved ones.

“The mom has been brought to tears and is beyond grateful for this experience.”

Because of this support, a child and her family were able to step outside of medical routines and simply be together—celebrating life, love, and a birthday that truly reflected her spirit.

These stories reflect the impact of the Foundation’s mission‑aligned support delivered through Lighthouse of Hope.
