The National Alliance for Care at Home celebrates the essential work of its members and leaders across the care at home community. Nationwide, Alliance member leaders are innovating, growing, and leading the future of home-based care, working toward an America where everyone has access to the highest quality, person-centered healthcare wherever they call home.
Seven Prestigious Awards
Lifetime Achievement Award
This award recognizes a champion and advocate, whose exceptional dedication and passion for home-based care have led to significant and lasting contributions to the patients, families, and communities served. Presented on a select basis, it honors individuals or programs whose leadership, innovation, and achievements have advanced care at home in meaningful and enduring ways.
Evaluation Criteria
- Dedication to Care at Home: Demonstrated passion and sustained commitment to delivering or supporting care in the home setting. Depth of involvement with patients, families, and communities over time.
- Leadership and Advocacy: Evidence of thought leadership, organizational influence, or advocacy on behalf of home-based care. Role in driving positive change
sat the local, state, and/or national level.
- Innovation and Advancement: Introduction or support of innovative practices, models, or programs that have improved quality, access, or delivery of care in the home.
- Impact and Legacy: Measurable and sustained contributions that have meaningfully improved outcomes or experiences for patients and families. Enduring influence on the field, community, or policy environment.
- Inspiration and Influence: Ability to inspire others, serve as a role model, or build momentum for the future of home-based care.
Above and Beyond Award
This award celebrates a dedicated, care at home professional who consistently goes above and beyond in furthering the provision of home-based care. With compassion, reliability, and an unwavering commitment to those they serve, this individual embodies the heart of care at home, making a meaningful difference in the lives of patients and families every day.
Evaluation Criteria
- Compassion and Empathy: Demonstrates consistent and genuine care for patients and families, going beyond basic responsibilities. Provides personalized care that meets the unique needs of each individual served.
- Reliability and Professionalism: Consistently dependable in fulfilling duties and maintaining a high level of professionalism in all aspects of the role. Demonstrates punctuality, accountability, and delivery of high-quality care.
- Commitment to Service: Shows an unwavering dedication to the mission of care at home, including a strong work ethic and willingness to go the extra mile for patients and their families.
- Impact on Patients and Families: Creates meaningful connections with patients and families, significantly improving their quality of life through the care provided. Receives positive feedback from patients, families, or colleagues.
- Collaboration and Teamwork: Works well with the care team, demonstrating a spirit of cooperation and mutual support to ensure the best possible outcomes for patients.
Volunteer of the Year Award
This award recognizes an outstanding volunteer or volunteer team whose contributions have made a meaningful impact on care at home. Whether through direct support to patients and families, organizational services such as fundraising and outreach, or specialized programs, these volunteers embody a spirit of service, compassion, and dedication.
Evaluation Criteria
- Impact on Patients and Families: Demonstrates compassionate, meaningful, and consistent support for individuals receiving care at home.
- Scope and Quality of Service: Provides direct or indirect volunteer services that enhance care delivery, such as companionship, administrative assistance, outreach, bereavement, or complementary therapies. Displays a high level of commitment, dependability, and professionalism in their volunteer role.
- Advocacy and Outreach: Helps raise awareness of the value of care at home through community engagement, education, or advocacy efforts.
- Collaboration and Teamwork: Works respectfully and effectively with care teams, staff, other volunteers, and families.
- Longevity and Dedication: Demonstrates sustained volunteer service over time, reflecting dedication to the mission of home-based care.
State Association Award
This award honors a state association in home-based care that has demonstrated exceptional leadership, innovation, and commitment to advancing care at home. Through impactful advocacy, strong member support, and initiatives that elevate quality, access, and awareness, the recipient exemplifies what it means to be a driving force for progress in the care at home community.
Evaluation Criteria
- Leadership and Commitment: Exemplifies commitment to serving as a leader in the care at home space.
- Innovation: Demonstrates innovative approaches to expanding and improving care at home
- Advocacy and Outreach: Helps raise awareness of the value of care at home through community engagement, education, or advocacy efforts. Positively represents the organization’s mission and values in public or support settings.
Care at Home Partner Award
This award recognizes a member or non-member organization that has shown outstanding support for advancing care at home through advocacy, innovative initiatives, or meaningful contributions to the organization’s mission.
Evaluation Criteria
- Outstanding Support: Demonstrates commitment to supporting the advancement of care at home.
- Innovation: Demonstrates innovative approaches to expanding and improving care at home.
- Advocacy and Outreach: Helps raise awareness of the value of care at home through community engagement, education, or advocacy efforts. Positively represents the organization’s mission and values in public or support settings.
Advocate of the Year Award
This award honors an individual who has demonstrated outstanding dedication to advancing the care at home mission through advocacy, public policy, and grassroots engagement. Through passionate leadership, strategic influence, and a relentless commitment to improving care for patients and families, the recipient has elevated the voice of care at home at the local, state, and/or national level making a lasting difference in the future of the field.
Evaluation Criteria
- Advocacy and Outreach: Helps raise awareness of the value of care at home through community engagement, education, or advocacy efforts. Positively represents the organization’s mission and values in public or support settings.
- Impact: Demonstrates innovative approaches to advocacy, public policy, and grassroots engagement that deliver meaningful impact to the care at home community
Dr. Bernice Catherine Harper Trailblazer Award
Dr. Bernice Catherine Harper is a pioneering leader whose work has had a profound impact on social work, hospice care, and equitable healthcare access. Rising from a segregated upbringing to earn a Master of Science in Public Health from Harvard, her legacy includes expanding access to hospice care through Medicare, establishing international care initiatives across sub-Saharan Africa, and championing person-centered care for underserved communities. This award honors individuals who embody Dr. Harper’s lifelong commitment to breaking down barriers and promoting access to care for vulnerable and underserved populations. Honorees demonstrate innovation, compassion, and leadership in expanding health equity and ensuring that individuals and communities historically marginalized by race, income, geography, disability, or other factors receive high-quality, culturally respectful, and person-centered care.

- Promoting Access to Vulnerable and Underserved Populations: Demonstrates commitment to supporting the advancement of care at home.
- Innovation: Demonstrates innovative approaches to expanding and improving care at home.
- Compassion and Empathy: Demonstrates consistent and genuine care for patients and families in the care at home community.
- Impact: Establishes methods of supporting historically marginalized or vulnerable populations that deliver demonstrated impact to the care at home community.
