Carmen Perry, Vice President of Provider Engagement 

As Paradigm’s VP of Provider Engagement, Carmen Perry brings more than 20 years of experience in the healthcare industry, including 15 years dedicated to assisting our nation’s veterans. A Certified Senior Advisor (CSA), Carmen is widely recognized for her strong relationships and deep ties across the healthcare and senior services industries. Her expertise in strategic planning, operational management, and business development enables her to drive organizational growth and deliver outstanding results.  

Outside of her professional role, Carmen is deeply committed to community involvement and advocacy in senior care. She serves as Chair of the Board of Directors for the National Aging In Place Council, is a Board Member for IDEAL for Healthcare, and also serves on the Board of the nationally acclaimed CSA Journal. Carmen believes strongly in giving back and is passionate about making a meaningful, lasting impact on the lives of seniors beyond her professional responsibilities.

Jacob Taitel, DMA Director of Strategy & Growth 

As a Medicaid and VA expert at Paradigm, Jacob has consulted with hundreds of home care providers across the country and is widely considered a subject matter expert when it comes to Third-Party Payers in home care. He thrives on building impactful relationships with agency owners that want to achieve sustainable growth through increasing their VA, Medicaid, or Medicare Advantage censuses. Although a far cry from his time spent as a professional tuba player, Jacob now enjoys reading, throwing pottery, and tending to his plants.
