(Alexandria, VA and Washington, DC) – The National Alliance for Care at Home (the Alliance) supports efforts by the Centers for Medicare & Medicaid Services (CMS) to combat instances of fraud, waste, and abuse in home health, hospice, and Medicaid home and community-based services (HCBS). This testimony before the House Energy & Commerce Subcommittee on Oversight and Investigations underscores the shared commitment of CMS and legitimate providers to eliminate bad actors and reinforces that continued, targeted action is needed to protect the integrity and sustainability of our federal programs.  

The Alliance continues to urge Congress and CMS to pursue enforcement strategies rooted in risk-based oversight—identifying and removing fraudulent entities without disrupting the legitimate providers delivering high-quality care in the home.  

CMS Deputy Administrator and Chief Operating Officer Kim Brandt’s testimony highlighted the administration’s ongoing initiatives to improve monitoring and oversight of fraudulent claims across Medicare and Medicaid. The Alliance is committed to these shared goals, and in late 2025, submitted detailed recommendations to CMS outlining data-driven, risk-based strategies to strengthen program integrity in Medicare home health and hospice. In early 2026, the Alliance also submitted a comprehensive set of program integrity recommendations to CMS Administrator Dr. Oz in response to increasing concerns around instances of fraud in Medicaid-funded services.

Home-based care plays a vital role in improving outcomes, reducing hospitalizations, and honoring patient choice. The overwhelming majority of home-based care providers deliver high-quality services to those who need them, in compliance with Medicare and Medicaid law and regulations. Any program integrity reforms must be carefully designed to remove bad actors without creating barriers for those providers operating in good faith and the patients and individuals who depend on them.  

The Alliance remains ready to serve as a resource to policymakers and regulators as program integrity efforts continue and is committed to collaborating on solutions that strengthen accountability while preserving access to care at home.
