2025 National Alliance for Care at Home Awards Program
The National Alliance for Care at Home celebrates the essential work of its members and leaders across the care at home community. Nationwide, Alliance member leaders are innovating, growing, and leading the future of home-based care, working toward an America where everyone has access to the highest quality, person-centered healthcare wherever they call home.
The Alliance is excited to launch its inaugural awards nomination process for 2025 and begin the process to select its first class of visionary leaders shaping and driving our shared work. We are now accepting nominations for outstanding individuals, organizations, and initiatives that exemplify excellence in our industry.
Seven Prestigious Awards
Lifetime Achievement Award
This award recognizes a champion and advocate, whose exceptional dedication and passion for home-based care have led to significant and lasting contributions to the patients, families, and communities served. Presented on a select basis, it honors individuals or programs whose leadership, innovation, and achievements have advanced care in the home in meaningful and enduring ways.
Above and Beyond Award
This award celebrates a dedicated care in the home professional who consistently goes above and beyond in furthering the provision of care in the home. With compassion, reliability, and an unwavering commitment to those they serve, this individual embodies the heart of care at home, making a meaningful difference in the lives of patients and families every day.
Volunteer of the Year Award
This award recognizes outstanding volunteer or volunteer team whose contributions have made a meaningful impact on care in the home. Whether through direct support to patients and families, organizational services such as fundraising and outreach, or specialized programs, these volunteers embody the spirit of service, compassion, and dedication.
State Association Award
This award honors a state association that has demonstrated exceptional leadership, innovation, and commitment to advancing care at home. Through impactful advocacy, strong member support, and initiatives that elevate quality, access, and awareness, the recipient exemplifies what it means to be a driving force for progress in the care at home community.
Care at Home Partner Award
This award recognizes a member or non-member organization that has shown outstanding support for advancing care at home through advocacy, innovative initiatives, or meaningful contributions to the organization’s mission.
Advocate of the Year Award
This award honors an individual who has demonstrated outstanding dedication to advancing the care at home mission through advocacy, public policy, and grassroots engagement. Through passionate leadership, strategic influence, and a relentless commitment to improving care for patients and families, the recipient has elevated the voice of care at home at the local, state, or national level making a lasting difference in the future of the field.
Dr. Bernice Catherine Harper Trailblazer Award
Dr. Bernice Catherine Harper is a pioneering leader whose work has had a profound impact on social work, hospice care, and equitable healthcare access. Rising from a segregated upbringing to earn a Master of Science in Public Health from Harvard, she broke barriers and built a career defined by compassion, resilience, and advocacy. Her legacy includes expanding access to hospice care through Medicare, establishing international care initiatives across sub-Saharan Africa, and tirelessly championing person-centered care for underserved communities. This distinguished award honors individuals who embody Dr. Harper’s lifelong commitment to breaking down barriers and promoting access to care for vulnerable and underserved populations. Honorees demonstrate innovation, compassion, and leadership in expanding health equity ensuring that individuals and communities historically marginalized by race, income, geography, disability, or other factors receive high-quality, culturally respectful, and person-centered care. As we celebrate Dr. Harper’s enduring legacy, this award reaffirms our collective responsibility to advance justice and inclusion in health care and to uplift those who bring care to those who need it most.
Nomination Details
The nomination process is open until September 4, 2025. All Alliance members are encouraged to nominate deserving leaders from their organizations. Nominees must belong to an Alliance member organization. Winners will be announced at our 2025 Annual Meeting and Exposition.
