Providing patient-centered end-of-life care to residents of facilities is an important aspect of many hospice organizations’ services. Facility residents represent a unique population and providing hospice services in a facility differs broadly from providing hospice care in the home setting.

Developed by a combined workgroup of Regulatory and Quality and Standards Committee members, Partnership in Care: Best Practices for Hospice Provider and Facility Partner Collaboration is a fillable tool intended to facilitate collaboration between hospice and facility staff, and to provide a framework to guide conversations between hospices and their facility partners to establish best practices in caring for their shared patients.
