Shepherd Home Health & Hospice

The home care team is made up of Registered Nurse Case Managers, LPN’s, Medical Social Workers, Occupational, Physical and Speech Therapists, and Home Health Aides. Patients are also supported by an Infusion Pharmacy, Telehealth Monitoring, durable medical equipment and community health services.

We provide a comprehensive variety of in-home services, collaborating with the patient’s physician and other disciplines to develop a plan of care for each patient. Our staff are carefully screened and trained so patients and families feel comfortable, safe and secure as they care for you in the home.
