Health Care Home – also known as medical home, patient-centered medical home and advanced primary care – as a way of delivering health care. It is an approach to primary care in which patients, families, primary clinicians and care teams work in partnership to improve health outcomes and quality of life for individuals and families living with chronic health conditions and disabilities.
Health Care Home is designed to enable better access to health care, increase satisfaction and improve health through comprehensive, collaborative care that puts the patient and family at the center.
All 20 Park Nicollet Clinic locations are certified by the Minnesota Department of Health as Health Care Homes.
