Home Care
At Community Medical Centers we offer a full continuum of care, including home health and palliative care. Our goal is to support both you and your family – in the comfort of your own home – by offering an alternative to frequent and prolonged hospital stays.
In the privacy of your home, we offer skilled nurses, therapists (physical, speech and occupational,) nursing aides and medical social services. All services are provided under the direction of your physician, as an extension of the physician’s care. The duration and frequency of our services are determined by your individual needs, and we’re available 24 hours a day, seven days a week.
Home Infusion
Our pharmacy staff educate, support and provide quality infusion care. The therapies we provide in-home would otherwise require hospitalization. Our services can also be provided in long-term or skilled nursing facilities.
