DC Function Measure Resource 

The Centers for Medicare & Medicaid Services (CMS) has posted the Discharge Function Score (DC Function) Quality Measure Overview in the downloads section on the Home Health Quality Measures webpage. This document is intended to provide Home Health providers with information on the Discharge Function Score (DC Function) quality measure.

This measure is included in the HHQRP and the expanded Home Health Value-Based Purchasing (HHVBP) Model and is now available on the home health iQIES reports for the HHQRP.
